Please do not go here. I have gerd so bad that I’m on 600 mg of Prilosec a day and aspirate my food about twice daily until I choke for 30 minutes. The doctor said I might not even need Prilosec after the surgery and this woman flunked my psych eval. With the excuse that she wanted me to see a psychiatrist and change my Zoloft medication I’ve been on for 18 years and told her it worked well and I’m happy. In 30 minutes she knows me better than 30 years of psychotherapy and psychiatrist. The single most unprofessional ridiculous appointment I ever had. I thought I should write this to save someone else.

I have referred several patients to Evolutis for pre SCS evaluations and have had very good feedback. Patients have found it very easy to undergo the eval in the comfort of their own home (especially important here near chicago where weather can easily interfere with a consult). Patients have also mentioned that their consults were performed in a concise and professional manner which I was pleased to hear.

My staff have found it easy to setup as well. They simply email the referral and Evolutis takes care of the rest of the setup.

What I personally like most about this service is the speed of turn around. I am usually surprised to see the completed report on my desk. Good time also to mention that the report is thorough and well organized.

The virtual appointment went very well and was so convenient! Didn't have to go in to a doctors office to wait! The dr was very pleasant to speak with and it didn't really feel like a psych assessment to prepare for my surgical procedure. The only reason I didn't do 5 stars is because I need this turned in ASAP to get my procedure done before the end of the year and it's going to take about a week; however, I realize they are busy and do their best I'm sure! Would very much recommend!

Getting very suspicious of this company. Attempted to make payment on website unsuccessfully. Get a call from someone in billing, give them my card number. Then they call back and say they got the card number wrong. Starting to wonder if this is a scam or just plain poorly managed. Anyone else have issues with this company?

We are so glad we found Evolutis Health!! In addition to offering virtual visits with flexible scheduling and an amazing turn-around time on their reports, their clinical psychologists provide comprehensive pre-surgical psychological assessments. I highly recommend this company!!

I could not be more happier with this company. I was hit by a semi truck 4/22/19.

It is a journey for sure.! Still with cognitive, memory issues, and fear of semi trucks which is impossible for me to avoid where I live. Thank God I have someone to drive, I just am so scared.

The help I am getting from the staff at this company is my life saver. I still have a long ways to go and with this companies caring, thoughtfulness, and help I feel I WILL get better in time and that means the world to me.

My only regret is that I didn’t find them earlier.
